YOUR AGREEMENT WITH CEO WORLD TRAVEL, LLC OR HARLEY HULA COMPANY, LLC

Payments: We require a deposit at the time of booking. If you pay by credit or debit card, you must complete the attached Credit Card Authorization. We will charge the deposit to the credit card entered during the booking. We cannot be responsible for cancelled reservations due to declined credit or debit cards, exceeding daily limits, or if the cruise line/tour operator rejects the card for any reason. We will try to reach you at the phone number, or email address on file for other means of payment. We will automatically charge the final payment on or about the final payment date in your confirmation. There is no need to contact us if payment is to be made on the same card as the deposit. If you are using a debit card, be aware that many banks have daily limits for withdrawal. We accept all major credit cards, as well as, checks (personal checks are only accepted 90 days or more prior to the travel date), certified checks, and money orders. Changing the credit card information does not automatically change payment methods on previously booked vacations. Please contact the accounting department at 609-707-4427 to change payment method. We cannot be responsible for any overdrafts or bank charges due to charging an incorrect card. Travel Documents: Thoroughly review tickets in their entirety. Verify all passenger accommodations, ascertain that all names are correct, and match the documentation that you will be using for identification. Many travel suppliers require preregistration forms completed online or via fax. These forms must be completed prior to the vacation; it is your responsibility to make certain this has been done prior to your vacation. Suppliers may charge a substantial fee plus applicable fare increases for changes made after the tickets or documents are issued.

Changes and Cancellations: We charge $50 per person for changes, including name changes; this fee will be in addition to any cruise line or tour operator charges. Notify us immediately if changes or corrections are required. We also charge $50 per person for all returned checks and all late payments. If you cancel, we will charge a $75 fee. This is in addition to any penalty from the cruise line or tour operator. Please read the travel supplier’s brochure or website for penalties. You assume full responsibility for those penalties. All requests to cancel a reservation must be forwarded to us by letter, fax, or email along with authorization to charge the $75 cancellation fee.

Responsibility: CEO World Travel acts only as a sales agent for any airline, hotel, car-rental company, tour operator, cruise line, or other service provider named in your itinerary, or for any consolidator named below (“Suppliers”). CEO World Travel is not responsible for acts or omissions of the Suppliers or their failure to provide services or adhere to their own schedules.

Release: CEO World Travel assumes no responsibility for and shall not be liable for any refund, personal injury, property damage, or other loss, accident, delay, inconvenience, or irregularity which may be caused by: (1) any defaults, wrongful or negligent acts, or omissions of the Suppliers; (2) any defect in or failure of any vehicle, craft, equipment, or instrumentality owned, operated, or otherwise used or provided by the Suppliers; or (3) any wrongful or negligent acts or omissions on the part of any other party not under CEO World Travel’s control. You hereby release CEO World Travel from all claims arising out of anything covered by this paragraph. CEO World Travel has no special knowledge regarding the financial condition of the Suppliers, unsafe conditions, health hazards, weather hazards, or climate extremes at locations to which you may travel. For information concerning possible dangers at destinations, CEO World Travel recommends contacting the Travel Warnings Section of the U.S. State Department at (202) 647-5225 or http://www.travel.state.gov/ For medical information, CEO World Travel recommends contacting the Centers for Disease Control at (877) FYI-TRIP or http://wwwn.cdc.gov/travel/default.aspx You assume full and complete responsibility for checking and verifying any and all Passport, visa, vaccination, or other entry requirements of your destination(s), and all conditions regarding health, safety, security, political stability, and labor or civil unrest at such destination(s). You hereby release CEO World Travel from all claims arising out of anything covered by this paragraph. You agree that the courts in Chester County, Pennsylvania, will be the exclusive jurisdiction for all claims brought by you or CEO World Travel, and you hereby submit to the personal jurisdiction of those courts. Insurance and Credit Card Protection: For your protection, we strongly recommend that you purchase trip cancellation insurance and that you check the appropriate box below. If you check “do not wish” or if you do not check a box, we will assume that you are not interested in travel insurance. We also strongly recommend that you use a credit card for your purchase, so that you can exercise your rights under the Fair Credit Billing Act if you do not receive the services you purchased.
